New Patient Registration
The partnership is happy to accept new patients from the Sandwell area. Newly registered patients will be given a questionnaire and asked to attend for a simple health check prior to been registered.
Registering Serving Armed Forces Personnel
The Practice will register serving members of the armed forces who have received written authorisation from the Defence Medical Services (DMS) to receive primary medical services and are currently living or working in the area covered by the Practice for the period where authorisation has been given.
Registering With The Practice If You’re Homeless
You are able to register with this practice even if you do not have a permanent home address. In this case we will register you under the practice address. You will be able to access information by regular attendance at the surgery, text messaging, or by any other means that are appropriate for yourself.
Moving Address
Please let us know if you change address or telephone number, it is very important to let us know straight away.
