Potential Relocation of West Bromwich Partnership for Health 
We want to assure you that you and your health and welfare is our number one priority. When we have discussed the potential move with our current PPG members and handful of registered patients the below questions were raised and answered.

Frequently asked questions:

1) Is Dr Tariq moving with you?
Yes. Dr Tariq and the whole team will be moving to the Lyng Centre

2) Do i need to re-register?
No, none of the patients will need to re-register, all your records will be transferred to the Lyng Centre so there will be no disruption with your care.

3) Is there parking available? What about disabled parking?
Yes, there is a large amount parking and disabled parking spaces available

4) When are you moving? and why?
If the relocation is approved we aim to move in March 2021. Further details will be available as they approved. We are looking to move due to the current lease on the premises is coming to an end.

5) I have a wheelchair, can i still gain access?
Yes, the Lyng Centre has wheelchair access, and there is a lift available to the second floor.

6) How far away is the Lyng Centre?
The Lyng Centre is a few minutes walk from the current premises on Izons Road. A location map of the existing location and the new location can be seen below.
